During the Brown v Mississippi case, the judge stated that a confession should be examined to identify whether the confession was made voluntarily or invol untarily and whether the confession was reliable enough (Scheb, 2010, 497). The ruling in this case was reversed because the police had brutally interrogated the suspect due to this the suspect had confessed to commit the crime. One way of approaching any confession and/or admission made by a suspect s referred to as due process voluntariness approach. According to this approach if a suspect confesses to be guilty if police officials have used coercion while interrogation, the suspect’s statement of being guilty is not accepted during the proceedings of his case. For example: in Arizona v Fulminante case, it was found that an undercover federal law enforcer coerced Fulminate and then he confessed to being guilty of first degree murder (McCord, 2012, 419). In this case coercion was not physical in nature; rather the coercion was emotional in nature as the undercover federal agent assured to protect Fulminate in prison as there was threat to his life in prison. The undercover a gent made the promise to protect while asking for a confession in return. This case was treated under the 14th amendment clause of due process and the court ruled in Fulminate’s favour, accepting that coercion was applied to obtain a confession. System Thinking In Healthcare Nursing Essay The health care system can be defined as a set of interrelated parts or agents, which include caregivers and patients, bound by a common purpose and acting on their knowledge. This great number of interconnections within and among makes the healthcare organisation complex (IOM, 2009). Such complexity brings problems and opportunities and requires organisations to adjust to the changes. The ability to understand and respond to both the external and internal environments might require a holistic thinking approach of the system (Lebcir, 2006). Systems consist of interrelated, interacting and interdependent parts configured in a manner that produces a unified whole. System thinking studies these components parts, their interrelationship and the way they function as a whole. According to Senge (1993), system thinking is a conceptual framework, a body of knowledge and tools that has been developed to make the full patterns of systems clearer, and to help see how they can be changed effectively. There are various system thinking approaches and the essay will review some methodologies that were used in managing the case study of Ashford hospital. 1.1 Case Study- Ashford Hospital Earlier in the year, Ashford hospital which serves quite a large population experienced severe pressure on service. The hospital had 67 ward beds. Patients had to wait for long in chairs or trolleys at accident and emergency unit (A E) before they could be admitted into the wards. This led to overcrowding of AE unit. Patients were asked to stay away from the hospitals AE unless absolutely necessary. The healthcare providers were put under pressure and resulted in trading of blames amongst them. Patients were no longer satisfied with the quality of care. The management of the hospital was disturbed and wanted a way out of the messy situation. 2 Soft Systems Methodology (SSM) SSM is an action oriented approach for tackling perceived real world problematic (social) situations ( Checkland and Poulter, 2006). Appendix A shows the SSM process steps that were followed in the course of investigation. 2.1 Finding out An investigation team was invited by Chief Executive (CE) and introduced to some health workers. Using the SSM the first stage was to identify and provide a brief description of the situation. Due to the workload the clinicians were encountering, getting them round a discussion table wasnt easy. However, the investigation team moved around asking questions and observing proceedings. A rich picture was developed to help capture the main entities, structures and view points in the problem situation of Ashton hospital (Figure1, Appendix B). As part of the finding out, the team had to identify key roles that were affected in this situation (Analysis 1). The team already knew who the client was because it was the CE who requested for intervention. The nurses and doctors (some with specialisation) in A E provided treatment to patients with various illnesses and injuries.. Where necessary, patients were moved to the ward. The bed manger allocated beds to patient, while the ward manager supervised the ward. Table1 shows the outcome of Analysis 1 while table 2 shows the worldviews of the issue owners. Having known the key issue owners, the social texture (Analysis 2) of the issue owners in term of their role in the hospital, the norms (expected behaviour associated with such role) and the values (standard by which behaviours are judged) were identified. This is illustrated in table 1 of Appendix B. A political analysis (analysis 3) which enriched the cultural appreciation previously obtained through Analyses 1and II was done. The essence was to find out the disposition of power associated with the roles within the hospital thereby buttressing our cultural understanding of the situation. The CE, being the head of the hospital, had positioned power over other roles while the doctors enjoyed expert power across the hospital. The details of the analysis 3 are shown in figure 2 of Appendix B. The culture analysis provided a basic for identifying the relevant issues, actors and conflict in the hospital. According to Checkland and Poulter (2006), every human situation reveals people trying to act purposefully. The models of purposeful activity system viewed through the world view of the doctors and the bed manager were considered very relevant. This was because the doctors made decisions on patients that needed admission while the bed manager was involved in allocation of beds. In order to model the purposeful activities, root definitions describing the primary activity processes and functions were developed using a mnemonic CATWOE analysis. Appropriate root definitions for the primary functions performed by doctors and bed manager were formulated as follows: A doctor system to provide quality and effective treatment care, through the use of appropriate acquired knowledge and hospital resources in, order to improve patients condition. The most significant finding that resulted from the investigation was in the area of discharge. The actual problem which was assumed to be limited to the AE was actually as a result of failure to adequately plan discharge in the wards. When the rate of patients needing admission increased, there was a need to change discharge plans. However, it was realised that the doctors did not change discharge behaviour and created waiting lists for patients that needed admission. Facilitated brainstorming sessions resulted in identifying number of contributing causes of delayed discharge. Discharge was done after ward round which took place in morning during week days alone. Insignificant numbers of discharge were done over the weekend because there was no major ward round. This meant that most patients needing admission in A E over weekend had to wait till following week before beds could be arranged for them. This also compounded the bed crises. A discharge project team was immediately set up. The main function of the discharge team was to carry out additional ward rounds in the evenings and on weekends so as to discharge patients and free up more beds. They were able to indentify other causes of delayed discharge and resolved them. This ensured timely discharge fashion which then freed up beds for patients in AE. 2.4 Strength and weakness of SSM The methodology provided guidelines that were flexible to apply. The use of models provoked debate and learning among the issue owners. Through discussion and debates, the hospital was able to realise that there was a need to amend patient discharge policy. However, this methodology could not satisfy everybody. Some of the discharge decisions were not favourable to the ward patients. Also members of the discharge team had extra work to do and ways of compensating them were not discussed. This could be seen from Jackson (2000) arguments that SSM tends to favour the more powerful people in the system while genuine participative debate could be severely constrained. 3.0 Thinking Differently Most of the inventions in our society today, such as electricity, telephone, automated teller machine and many more, are the results of some people who decided to think differently. Thinking differently involves using innovative and creative approaches to transform healthcare delivery service (NHS, 2007) The first stage was to stop and think of the whole situation and identify areas where creative thinking could improve matters. It was observed that there was poor co-ordination of patients and beds management while poor communication existed between the bed manager and other clinicians, in the wards and A E, about bed availability. A tool called Others Point of View (OPV) was then selected to describe the issue from others peoples perspective. The aim was to generate some alternative ways of framing the problem and to think about what other people might say about bed management of the hospital. This gave us insight to how clients and resources were being managed in other industries. The stake holders then agreed to try out a bed management information system. The hospital implemented bed management information system on a small scale and some of the benefits were highlighted (Table 5). However, this took time and caused arguments as feasibility, advantages disadvantages and risk of each idea were all argued out. Also, traces of people trying to impose their ideas on others were noticed while others brought up ideas that would satisfy their own interest. 4 System dynamics System dynamics is an approach and simulation technique for studying and managing complex feedback systems, that are seen in business and other social systems (SDS, 2009). According to Jun el at (1999), there has been increased use of simulation in healthcare. This could be attributed to numerous success reports of using simulation to address health care system problems and availability of simulation software packages. The methodology was used to estimate and manage the aggregate flow of patient through the hospital and its environment. As the number of people visiting AE increased, it in turn increased the number of people who needed hospital admission. Also, as patients waiting for bed increased, it caused an increase in number of patients that were discharged home early. This is illustrated in Figure 1. There was the need to control the number of patients coming through A E. The availability and use of other healthcare facilities such as GP practice, community care and nearby hospitals would have a negative feedback on the inflow of patients into aE. This would directly affect the number of patients needing admission. Most of the patients that were discharged home might not have to return to AE as community care could assist carter. This methodology helped the hospital to forecast inflow of patients and ways that could be adopted to control it. Community care services around were informed about the situation of things in the hospital. In addition, ambulance bringing patients were informed ahead to make use of other hospitals or GP practices if condition of patient was not too severe. The various methodology approaches used assisted in better understanding of the relationship between the various units in Ashford hospital and its environment. This showed that like any other system, no unit in the hospital is an island. The different methodologies applied in Ashford hospital situation yielded various positive outcomes that helped improved the problematic situation. While some of the methodologies used have their roots in action research, further participatory research will be carried out to investigate occupational work stress among the clinicians. This may help identify majors causes of stress associated with their workflow, how it affects their relationships with others and with the patients in particular. Word count -2034

However, philosophical scepticism involves more than this. Its essential element is a general view about human knowledge. In the broadest terms, philosophical scepticism holds, or at least finds irrefutable, the view that knowledge is impossible. There are two features of philosophical scepticism which differentiate it from everyday 'sceptical' outlooks. The first has to do with its strength. The more challenging sceptical arguments do not depend on imposing high standards for knowledge or justification. Rather, the scepticism they imply is radical. Globalization is a term used to explain the new era just like we learn about different periods such as stone age. Globalization can be found in five different areas; economic, cultural, political, religious and social systems. Globalization therefore, can be understood as the tendency or the process of businesses, technologies or philosophies to spread throughout the world. Nature of globalization Globalization is something that is very new in the world. For the last 30 years, there was no much discoveries in computer technology although there were computers and they were only used in the developed countries. This is why we cannot say there was globalization since globalization simply means making the world a â€Å"global village† in that every information and services are available and accessible to all the people around the globe just as it is in a small village. The idea of globalization has made it possible to harmonize the cultures of people all over the world. That is because people are able to carry out businesses across boundaries without traveling, using different currencies, making instant payment and in developed countries they use a common currency for example the Euro that is used all over Europe and the US dollar that is used by many countries. It is possible therefore, that the whole world will in future be using one common currency. This is what is referred to as economic globalization. According to Sachs (2005) technology in information has made transfer of information and especially educational information to be available across the globe. This is what has brought the idea of e-learning through the Internet. The changes in culture of the people is being felt especially in the African societies which had very stronghold in their traditions but today this has little or no influence on peoples ways of life especially in the young generation, for example changes in sexual inequality. This is as a result of globalization, people copy the lifestyles or cultures of others from the developed countries due to interaction, travels and high speed information transfer. For this same reason people are able to change their religious believes because the gospel has taken a new line, i. e. it is global reaching every corner of the globe. Is War Ever Justified Essay War, although being described by those who have survived it as hell, is in my opinion a necessary part of life in some sense in order to expand in many ways like socially, economically, and politically. In terms of social changes brought on by war, war often teaches us lessons about how to better our behaviour and attitudes in terms of our association both domestically and foreign. In World War 2 (1939 1945), America and Germany’s interaction and hostilities soon diminished after the end of conflict suggesting that they had learned to get along (or at the very least solve their issues in a more diplomatic way. ). Learning from our mistakes is essential for growing as a race in general. Had it not been for World War 2 the UN (United Nations) would have most likely not been formed and therefore our world would not feel the sense of unity that is present today. In terms of economic changes brought on by conflict or changes, had it not been for wars we would not gain items of value for the use of economic resources. Had the USA not gone over and improvised in Afghanistan they would have most likely lost what is estimated to have been one trillion dollars worth of oil. For this reason the Afghan War (2001 Present) has been referred to as the â€Å"resource war†. In terms of political change brought on by conflict or war, we as a society have learned many lessons of the importance of a strong political power which could in fact reduce the need for conflict or war. Had it not have been for the Vietnamese War (1959 1975), we would not have been shown the importance of international involvement from a single nation in order to assist a less fortunate nation. War has always, in the end, increased all nations sense of unity in some way. Had it not been for the Vietnamese War, America would still be in some state of singularity and arrogance. The loss in Vietnam for the Americans proved that even they the very powerful can be defeated; if they were not given assistance the losses could have been far greater. War may be brutal but there are many positives that can be taken away from it. War, though an expression of our inability to coordinate and understand one another is also a great teacher for the world at what is right and what is wrong. The atrocities and crimes committed during wartime, though terrible and evil as they appear are usually necessary and in the long run will eventually prove to be better for society. During World War 2 had the Allies not declared war against the Axis it is unthinkable as to what could have happened to our world. The Allies chose to stand up to what atrocities were committed by Hitler and stop his evil from spreading. World War 2 is also an exceptional example of what happens when good men fail to act and allow the actions of evil men to spread. Hitler rose to power and began breaching the Treaty of Versailles and built an army despite the Treaties clear clause in which it states that Germany is banned from ever creating an army to avoid the repetition of World War 1 (1914 1918). Had a militant force not attempted to stop Hitler and his Axis powers it is unthinkable as to what the outcome of Hitler’s plan could have been. World War 2 was completely justifiable in that the evil committed during this war far outweigh the possible evil that could have occurred had a war not occurred. Had a militant force not intervened and caused a war it is quite probable that Hitler would have just kept exterminating the Jewish population and that the world would have just continued to watch. During World War 2 we learned the importance of standing up against the tyrannical and not being afraid to stand up for what we as a civilisation believe to be acceptable behaviour. The conflicts during World War 2 could very well have been avoided had good men acted against Hitler’s breach of the Treaty of Versailles and had the courage to stop him before, as we can now see, his plan could have been put into motion. We can simply justify World War 2 in that had The Allies failed to intervene the Nazis would simply continue to exterminate the Jewish population and many attempts to reach Hitler on a political level had been attempted and failed. War, although being a major drain on our economy, is also a great representation of our ability to learn and adapt to what is occurring throughout our time. Had we not spent so much money on war machines during wartimes, we would not have defences in place today that protect our nations against attacks from other possible threats. It is also important to recognize that had wars not occurred it is quite possible that our economies would in-fact be damaged by this. for example a large portion of the economy revolves around oil and it is estimated that had America neglected to act in Afghanistan and stop the Taliban from burning their oil supply as an act of aggression and arrogance against the USA, it is possible that approximately one trillion dollars worth of oil would have been wasted. Had the USA not stepped in and intervened in this conflict, America’s already struggling economy would have been severely damaged and it is possible that this massive loss of oil would have quite possibly caused another depression in America. Had America not invested so much money into the conflict in Afghanistan there economy would have been severely damaged by this. The war in Afghanistan can be justified in that it was not originally intended for the sole purpose of protecting a means of economic revenue but was originally intended for the purpose of retrieving Osama Bin Laden and to punish him for his atrocities committed. It was only after this fact that Osama’s militia began to burn up the oil fields that American geologist discovered that Afghanistan was actually sitting over one of the worlds largest oil deposits. Many people who are against the war in Afghanistan claim that this conflict comments on Americas greed for Afghanistans oil deposits but it was intended originally to have been a mission to capture a known terrorist and to protect the people of Afghanistan. War is often a result of the lack of political effectiveness to eliminate issues and this causes violence. War can be justified in that many people are so close minded in that they would rather resort to violence and not accept a diplomatic solution to their issues that they have. The Vietnamese War is a result of America and her one-time ally the Soviet Union’s inability to discuss and resolve their issues without resorting to War and conflict. The collective inability of the Soviet Union and the Vietnamese Government to accept Americas attempts at peace resulted in this conflict and therefore this conflict was justified. The Vietnamese War was somewhat unavoidable in that many attempts by America were made to achieve peace with Vietnam. After the Vietnamese War was eventually lost by the American Military, it was determined that the original intention of the Vietnam War was to preserve the Vietnamese people’s right to self determination and freedom from oppression by the government. The war is justifiable in that America was merely attempting at preserving a god given constitutional right in that we as a people deserve the right to choose our actions without input from and political force. The Vietnamese War had the best intentions to preserve our rights but was perhaps ill prepared on part of the American Military. Sometimes in order to do whats right we have to put aside the safety of others and make the difficult decisions that need to be made. Each and every war or conflict throughout history has been caused by circumstances that require action. Had action been neglected during any of the wars listed within this paper the results of these wars would have been far worse and many more would have died. All of the wars listed within this paper were unavoidable and justified in the fact that they all had the best of intentions in mind and that they were all attempted to be solved through non violent means. Furthermore, CHT St John has a unit manager and a registered nurse, clinical coordinator, and health care staff support. In addition, quality activities were piloted and this produces improvements on their practice and service delivery (Ministry of Health 2014). The CHT St John conducting meetings to tackle quality and risk management process and residents meetings with their own families are conducted every year. Moreover, the documentation evidence that is related to infection control education is provided to the service providers as a part of the orientation and as a part of an ongoing in service education program. Furthermore, the type of surveillance carry out is suited for the organization as to its size and complexity and standardized definitions were utilized for the identification and classification of infection events. The results of the surveillance were acted upon, assessed and reported to the relevant person in a well-timed manner (Ministry of health, 2014). Recommendation In CHT St. John Hospital certificate of audit is done last September 2015 which they have fully attained the different areas conducted by the Health and Disability Auditing New Zealand. This shows that the organization had achieved a success in providing quality of care to their consumers. Nevertheless, it is recommended that CHT St John Hospital should expand more their services like putting up additional building so that it could cater more patients to care. The argument is not black and white, rather, it encompasses many shades of gray. At the one end of the spectrum you have the National Rifle Association (NRA) which currently views any type of gun control as an infraction against the Second Amendment of the Constitution ("What is the NRA" 1). At the other end of the spectrum you have groups like the Coalition to Stop Gun Violence (CSGV) and Handgun Control, Inc. seek to make most firearms accessible only to law enforcement and the military("CSGV" 1). In the direct meaning, â€Å"to medicate† is defined in Free Online Dictionary as â€Å"treat with medicine†. This meaning is widely accepted when the word is used in a general sense to refer to the idea of the process of treating medical illness with substances.In many cases, however, the word â€Å"medicate† takes on an additional meaning when it is used to denote the process in which people try to use medical methods in a random, casual way to solve serious problems. One can try to medicate a serious disease resorting to folk ways to medication, or treat a condition with ma gic or witchcraft.â€Å"To medicate† has become popular in today’s fast-paced society where people are tempted to jump to easy measures to ward off the constantly increasing stream of problems. This effort gave the word an additional connotation of a â€Å"quick fix†.This additional shade of meaning dramatically expanded the original meaning of the word. Now the action signified with â€Å"medicate† no longer needs to refer to those moves that involve medical substance. One can take drugs to medicate a love failure, or get a cup of morning coffee to drive stress away.Carl Eliott in his essay â€Å"Medicate Your Dissent† applies the word to the spreading inclination of many Americans to turn to antidepressants when they want to correct their depressed state. More often than not, they are penalised for their self-confidence.This is why â€Å"medicating† often has disastrous consequences. A person can be assured that everything is going well, and he or she is on the way to recovery, while in fact the disease or other problem is growing into an even bigger one. Temporary solutions can place human mind in a state of blissful unawareness when a person revels about a problem being solved and fosters passivity with regard to real issues.In my experience, the most vivid example of â€Å"medicating† in th e latter sense of the word was a married couple that tried all kinds of short-term solutions to a problem they had. The wife moved to her husband’s place of residence in rural Austria, having lived all her life in the US. Her urban background left her totally unprepared for life in a rural community in a foreign land where she did not know the language and felt that the local residents did not accept her.They tried all kinds of solutions that would temporarily solve the problem – she joined various local clubs, engaged in community life, tried to work as a freelance designer taking orders online.   In the end, like so many people trying to overcome their problems, she took to anti-depressants so as to remove her worries and concerns. Surely, anti-depressants did not save her marriage that ended on the rocks after barely two years of family life, after passionate dating and a honeymoon filled with explosive happiness.Kara (my friend’s name) realized too late th at she should not have entered this relationship at all, for although they were enthusiastic about each other, they were two different people with differing backgrounds, which made family life difficult if not impossible.No matter what she tried when she got to Austria, her inner strength and communication skills were not enough to make her life there not only enjoyable, but even tolerable. Instead, her attempt at medicating her pain over separation with her relatives and her native culture with anti-depressants gave a serious blow to her health as she developed side effects associated with the drugs.Thus, â€Å"to medicate† means to invent short-term solutions to long-term problems. â€Å"During peace time a scientist belongs to the world, but during war time he belongs to his country†-Fritz Haber Here lies the ashes of the famous chemist Fritz Haber and his first wife Clara. Fritz had a heart attack and managed to recover, but not fully. He died of heart failure on January 29, 1934 at the age of 65. This â€Å"Father of Chemical Warfare† is known for developing and deploying chlorine and other poisonous gases, the development of gas masks with absorbent filters, and trench warfare in World War 1.Even though he was too old to enlist in military service, this scientist was given the rank of captain. He was most known for the Haber process (the reaction of nitrogen fixation to industrially produce ammonia), the Haber-Weiss reaction (a reaction in cells that creates oxidative stress), and the development of several fertilizers and explosives. He was also given the Nobel Peace Prize in 1918 for synthesizing ammonia for fertilizers and explosives. In 18 98 Haber published a textbook on Electrochemistry, which was based on some lectures he gave.In 1905 he had published his book on the thermodynamics of technical gas reactions, in which he recorded the production of small amounts of ammonia from N2 and H2 at a temperature of 1000 ° C with the help of iron as a catalyst. This German chemist was proud of his work even when no one, including his family, stood with him. Clara and his first son later committed suicide because of the shame in what Haber did in chemical warfare, but even the lack of faith in his family didn’t slow him down. It states that the absence of rules may not provide accurate reporting of financial results. When strict rules are followed the accuracy increases and the possibility of ambiguity decreases. On the other hand, princ iple based accounting as known as Generally Accepted Accounting Principles is more focused on the conceptual basis for accountants (Bennett, Bradbury & Prangnell 2006). It is more concerned to ensure good reporting through a set of guidance. Principle-based accounting provides a broad range of guidelines that may be practiced for a variety of circumstances (Collins, Pasewark & Riley 2012). Accountants debate that the application of rule-based and principle-based accounting is as complicated as issues of economics; the dynamic economical conditions are triggering point for accounting circumstances to transform (Benston, Bromwich & Wagenhofer 2006). Therefore, the effectiveness of the application of these rules and principle may remain controversial. Considering the conceptual framework both accounting standards are based on simple mathematics. Treatment of Elements The treatment of the accounting elements is evidenced that the accounting principles are based on simple mathematics (Jo nes 2011). The treatment of assets, equities, liabilities, revenue and expenditures in both rule based and principle based accounting standards is alike. According to the conceptual framework, the objectives and elements of accounting principles are similar (Bennett, Bradbury & Prangnell 2006). Principle-based accounting and Rule-Based accounting have largely same objectives. The qualitative characteristics of both standards are also similar; according to the recent research it has been concluded that GAAP and IFRS follow basic agreements on the qualitative characteristic, that is, Relevance, Understandability, Reliability and Comparability (Collins, Pasewark & Riley 2012). Qualitative Agreement The concept of understandability is also similar in both standards. Principle based accounting and rule based accounting standards emphases that the financial information shall be understandable for users of financial statements (Ampofo & Sellani 2010). Another qualitative agreement, that is relevance, both the standards provide basic agreements on that the financial information should be relevant. If we focus on the treatment of elements in Principle-based accounting and rules based accounting is similar (Agoglia, Timothy & Tsakumis 2011). Consider the treatment of revenue in the both principles is similar (Whittington 2008). The foundation of accounting is based on a formula that is assets are equal to a sum of liabilities and equities (McGregor & Street 2007). No major differences in the treatment of as
